Our Patient Policy

Thank you for choosing Apple-A-Day Clinic as part of your healthcare team. We are committed to resolving your health problems. Please understand that payment of your bill is considered part of your treatment. The following is a statement of our financial policy which we require you to read and sign prior to treatment. All patients must complete our Patient Information Sheet before seeing the doctor. Please inform us of address or other changes at subsequent visits, if applicable.

 

Regarding Insurance

If you are referred to us by your insurance company, please call us at 847-577-4455 Monday through Friday from 2pm to 6pm. Otherwise you are responsible for your bill at the time of your visit. Please be aware that some, or even all, of the services provided may be non-covered services. You will be billed by credit card for all unpaid claims eight weeks after your appointment. We DO NOT accept Medicare. 

 

No Show

If you are unable to keep an appointment, we must have at least 24 hours notice. Any appointment missed or canceled with less than 24 hours notice will be billed for the appointment in full. We require you to provide a credit card or use the credit card on file below with the authorization to bill that account for the missed visit. New patients that do not call to cancel will not be rebooked under any circumstances.

 

Supplements

We require that you purchase the supplements prescribed from our office to receive the highest quality, most thoroughly researched and tested products.  Many products purchased online are fake, counterfeit, or have been sitting in a warehouse for an extended peroid. It also enables us to substitute another in cases of intolerance.  This will enhance the overall success of your treatment. Shipping is available for an extra fee.

 

Multiple Cancellations

We reserve the right to refuse to reschedule you if you cancel two or more appointments in a row. We reserve the right to refuse to schedule you for any reason
